‘SilverConnect’ is a three-year digital healthcare and intervention programme for seniors, funded by DBS Foundation and planned by the Hong Kong Council of Social Service (HKCSS). The programme aims to support seniors with varying health conditions to adopt healthy lifestyles and improve their quality of life through a three-level health management model. This is achieved by leveraging a ‘tele-practice platform’ and various gerontechnology solutions. At the same time, the programme also helps caregivers improve the quality of care. In collaboration with Community Partners, the programme provides seniors with health screenings and follow-up training for early detection and intervention, and supports professional therapists in incorporating tele-rehabilitation into standard practice to increase the effectiveness and frequency of home-based rehabilitation.
